oh cwillie, I would definitely make a full report on the dir of food services. You are right, how dare she! They need to have an in-service on those that are around in the dining rooms (food services, aides, etc.) when people eat and make note of anyone having difficulty swallowing... and reporting it !!!

They are part of the "nursing home team" and everyone has the responsibility of watching out for their residents! I would have fired back at her... why didn't you think it was a priority!!!! How many people don't have family that can sit with their LO during meals 3 x daily!!!

Paramedic wanting to haul mom said "WE care what happens to your mother!" Like I don't? BURN..... you don't want to hear what I said; not cussing, but not nice. I hate it that so many medical people think they are gods....

Oh too funny, yes I am never fluent of speech at the time, and think of things later in the day or even a couple of days later... makes me so mad! It's a case of being in disbelief... and then comes the not. being able to think of a come-back.

And the medical God-like thing... wow I can relate to that... I am an RN and have had to deal with that in my professional and personal life. Makes me so mad I could spit!

86* in the living room right now at 7 pm. Not bad if you don't move a muscle and sip an iced beverage. Bad part is, you get SO sleepy from the heat. Ceiling fans going 24 hrs/day. Phew!

I know I'll want this weather back when it's freezing (very cold) in the winter. Right now that is no consolation.

I am no longer sick but then DH got sick and was hospitalized briefly. I worry about him, he is usually the caregiver and I’m the disabled one. We wonder what’s ahead and he’s been so quiet since he came home from the hospital. He’s worrying what will become of us if we reach severe old age? We made it through these months with DH’s parents and my daughter’s help. It’s made me realize how my parents feel, my mom very fragile in assisted living and my dad still functioning and living at his/mom’s house, with one foot on a banana peel!!!

Reality check! We all do better just living one day at a time!

Our weather is normal on the Gulf of Mexico. No hurricane yet. In New Orleans on a doctor’s appointment Wednesday, it was 99 degrees with blinding sun and not a zephyr of breeze blowing. Just normal.

Impotent FURY!!!
The dietitian sat with us at lunch today and said she had given mom her breakfast and felt she had no difficulty with the honey thickened fluids. She also told me that she has studied this a lot and it is her belief that pudding thickened fluids are never beneficial because they tend to coat the throat and can themselves cause aspiration. I was more or less told I was welcome to bring in an outside S&LP at my own expense but that there was nothing I could ever say to get her to prescribe thicker fluid, ever.
So once again I am wrong and the experts are just doing their jobs. It is preferable to cough and gag your way through meals rather than aspirate later, right?

Does the dietician really have the last say in this?!? Not the Dr?

I think it was the Speech Therapy person that actually evaluated my Mom’s swallowing difficulties and made suggestions to her Dr and Dr in turn wrote the orders.

I realize you are in Canda but is the chain of command that different?

Dietitian has training in S&LP - good luck finding S&LP who will travel to small town and contradict a fellow professional, I expect doc will also defer to her. I've been looking online for any study or anything to corroborate this woman's belief but have found NOTHING. I don't think I have it in me to push this any farther, they have won, I am defeated.

Can you take a video on your cell phone of your Mom choking on her food, CWillie?
Even healthy people may cough, choke, or sometimes aspirate on occasion. imo.
I think anxiety can cause these episodes, but I am not sure and not an expert.

It adds to the feelings of helplessness when "the expert opinion" over rules common sense because they have studied it.

Fallback, regroup (only one person who cares) keep your eyes and ears open, and keep on watching out for your Mom. Continue to avoid that person, if possible.

I just don't think there is any point, every argument I give (about anything) is always countered and it becomes a continuing, drawn out battle. We are supposed to hand our parents (spouses, brothers/sisters, etc) over to their capable hands and trust them to look after them - as much as they try to deny it there is no protocol in place to take the wants, needs or opinions of family (or the residents themselves) into consideration if they are counter to the norm.

Not being heard, not listened to, dismissed can be some of the worst assaults on our being, our self-esteem.

I wasn't there, I'm pretty certain all she did was spoon some OJ into her and observe that she didn't choke on it. I mentioned that she didn't always have a problem but overall seemed to do better with thicker fluids - she didn't want to hear me because she had all ready made her mind up due to her personal prejudice against pudding thick fluids. I should have asked her to quote the studies she was referring to because I have searched and found absolutely nothing that backs her claims. I didn't stay for supper tonight because I will either cry or hit somebody - maybe I should just stay away and save myself the frustration.... they say ignorance is bliss.

In agreement you might need a break to counteract the excessive stress in staff relations, and to protect your own emotions. A short break.

Or visit after mealtimes, in berween mealtimes?

You know best, and you always keep the goal of Mom's care in mind.

A small break to refresh yourself?

Good idea, MsMadge, an exam to test swallowing muscles.
Is a dietician qualified to make such an exam?

Some professionals are clueless. One of the times when my dad was in rehab at an excellent facility he would cough when he drank water or coffee. The Speech Therapist came to look at him drink and he started coughing. The only thing she said was for him to swallow twice after drinking. All that college gone to waste. 

When he came home from rehab I told the nurse from in home support about his coughing and they sent a Speech Therapist to his house. He was given exercises and told he needed thickened liquids.

You know if I had been able to find even one reference to thickened fluids clinging to the mouth and throat and therefore causing a latent risk of aspiration I would be willing to at least consider her point of view but I can't find anything, not even on the kooky outer edges of the internet. It sounds plausible, but everything I'm reading tells me it isn't. How can I counter a belief in pseudo science?
