Ellis House, LLC was established in 2018 because of my late grandfather Ellis Poole who I share the honor of having his first name. My grandfather was a kind, loving, and giving man who volunteered his time helping family and friends as a POA at a time when they needed someone responsible to make thoughtful decisions about their care. He was the type of person that you could count on and loved to take care of his family circle. His values of serving others lined up with mine.
One of our goals is to aid Life Care Communities and Continuum Care Communities clinically manage their residents at home by preventing them from returning to the hospital after discharge from the skilled nursing facility within 30 days. Ellis House, LLC will partner with organizations by sharing clinical information from our Home Health Care team providing data that leads to working towards positive outcomes for the resident-client.
