I am contemplating hiring a nurse to do a home safety evaluation/visit, help with medical care coordination with the many different providers my mother has, and also reconcile her medications and fill a med planner weekly. Has anyone used this type of service? This is not Home Care as they do not provide any kind of clinical care. It is also a cash pay service. However, if she needs Home Care, therapy, or any other services, they can help coordinate, get insurance, approvals, and find local service providers.Please provide feedback on whether it was worth it, also the pros and cons. Thank you so much!

I have, and am currently utilizing a service that sounds like it is similar. It is a Care Management service. The woman I engage with (and she is employed by the actual small company) has been a nurse as well as a CNA in skilled nursing facility while she studied to become a nurse. They offer levels of service as well. We paid a large initial fee and they are allowed access to my father’s health records (he approved). She came to our house and met him. We engaged her because he started going in and out of the hospital with many complicated issues at the same time his much trusted of many years PCP retired. She was on hand to advise about many things, including which skilled nursing/rehabs were preferable on his list of choices should he need to go to one post hospital (as the service also visits other clients who are residents in nursing homes and monitors their care). We now pay an hourly rate based on time provided in fifteen minute increments, and contact her should the need arise.

Dad has most recently been in rehab at a SNF and she visits him there (and I am present as well), prompts me to ask appropriate questions that I might not of thought of and watch for things I would be unaware of, and is a tremendous support during health issues (in terms of advising me).

I know they offer more involved services for people who do kit have local support, like taking them to doctors appointments and taking notes during the appointment and even asking questions, I’ve known them to take some clients shopping, etc.

I have used a geriatric care services provider a few years ago but not extensively. The one I found offers needs assessment, referrals of all kinds — lawyers, geriatric doctors, home health care agencies, assisted living, memory care, rehab places, coordinating moves, medication management. etc. They have nurses, social workers, a psychologist on staff. Everyone I spoke to or worked with there seemed very knowledgeable and professional and caring. The owner was very clear about offerings and cost structure. She gave me a fair amount of general advice for free.

Their nurse came to check on my parents when I was traveling once, and later when my dad was in hospice, they found a male home health aide for my dad. Both of these people seemed great to me but my parents ended up telling them not to come back (“we don’t need help.”)

I agree with Geaton’s advice. Just because one of these companies is good or bad doesn’t mean they all are.

I would go on to Nextdoor.com and post this question. You will get better input because it's a company local to you so neighbors and others in your community may have experience with this specific company you are considering.

You don’t give any information about what sort of care your Mother needs. If this service does not “provide any kind of clinical care”, it’s hard to guess what the cost will be for their “help with medical care coordination”. Will it be a one off charge, or will it be ongoing?

Many older people end up with “many different providers”, and it makes a lot of sense to cut down the conflicting advice and multiple appointments. Will this service do that for you? Or what will it cost, and what will you get for the money?

I made a boo-boo when I posted this. I did not mean cash only. This is a legitimate company that does not accept insurance as payment. They will do a consultation over the phone and come up with a treatment plan. Your charges will be based on what is needed. I think they have like three levels to choose from. There are contracts to sign and receipts obtained. It is an actual company and they have a business license. I’m sorry if this was not made clear. So I would definitely like to hear from someone if they have ever used these types of services. It just doesn’t go through insurance.

Talk with her primary doctor.
Ask about palliative care and hospice care. Hospice care may have a social worker to help guide you in the right direction.

Hey there, thank you for your response. I misspoke by saying cash only. I meant to say they do not accept insurance as payment. It is a legitimate company and you pick what level of Assistance you need. You get receipts and they also have you sign a contract indicating what services will be provided, etc.
Thank you for taking the time to respond and keeping me safe. Also, no need for hospice as the service is just trying to keep my mom living independently as I am not local to her. She still drives and gets around, she just has trouble hearing what the doctor tells her to do and following through with medication’s and recommendations.

I agree.. if that is actually the way it sounds.. Cash is the only form of payment..no trails, no receipts?

Any service medical or not that are a "cash pay service" only would be a HUGE red flag for me, unless I'm missing something here.
Where will your paper trail be if only cash is being exchanged? It seems a bit fishy to me. I'm just saying.

Thank you for the reply and I actually misspoke. It is a company that just doesn’t accept insurance of any kind, not even Medicare. There are contracts to be signed and receipts are obtained. Thank you for having my safety in mind, I really appreciate it.
