If you don't want him home, then you need to find a permanent place for him to stay. Is he in Rehab now? If so do they have LTC at the same facility? If so, ask to have him transferred there. If you have no assets, then you apply for Medicaid. If you have assets, then an Elder Lawyer is needed to split those assets. His split going to his care and when gone, Medicaid is applied for.

You need to tell these people you cannot physically care for your husband, unsafe discharge. Speak with the Social Worker there.

Tell nurse/discharge planner that he is an "unsafe discharge" and then do not go to get him. Ask to talk to a hospital social worker and discuss transitioning him directly into a facility.

Hospitals will apply a lot of pressure to move patients out, so do not believe it if they offer to provide "help" once he's home. They won't.
