Where I live you could call your County Office of Aging. They may have a small bus that will take you to appts and shopping. Ours even offers outings. If there is a Senior Center near you, they may be able to help. Are you on Medicaid, they help with transportation. Ask your doctor if your therapy can be done in your home. He can right an order for it. While you are talking to Office of Aging, see if there are any resources you can take advantage of.

Look up and enroll in your county’s paratransit, usually run by the overall mass transit agency. Paratransit is door to door and if not free, very cheap.

You could check whether Medicare or your insurance or supplement will have the physical therapist come to your house.

You can contact your local social services and ask if they offer transportation services

This is a forum of individuals who are involved in caretaking for their family et al. You need a transport service. I’d start with your medical insurance.
