DebraAnn, your best bet is to work at a caregiving company as an employee, that way you can get some first hand knowledge and experience with working with all types of patients. Working with family, you are just seeing a tiny percentage of what is required.

Also, you will need to have a healthy savings account, as starting a business can be expensive. You need to know how to do background checks on future employees, and what State laws there are regarding caregiving. You would need a county/city business license, and possible a State license. Liability insurance will be costly, so speak to an insurance agent. Your employees would need workman's comp insurance. Then there is payroll. Clothing for your employees. An office to work out of, unless where you live allows you to have an at-home business where employees come and go.

There's always paperwork. I saw a lot of requirements for a company to provide personal care under a Medicaid program on your State's health and human services website. 

The rules may be different for self-pay agencies, but you'd still have to register with the State as a business, get a tax identification number from IRS, take regulatory/other courses...

You're doing a fine thing, wanting to provide excellent care. Getting through the legalities will be your first challenge.

Certified as a Certified CNA or RN. The one who can best answer somebof your questions would be Burnt. She started her own business a while back after being a CNA for 25 yrs. There will be alot involved.
