My Dad has been at a rehab facility after a stint at the hospital. He has 7 days left at the facility. Then we need to bring him home. More because since being in rehab he has gotten worse. Therapy informed me he could use the bathroom. the nurses/ aides informed me and him to just use the damn diaper. It is just horrible. He didn't arrive in a diaper they put him in one as soon as he arrived. Home is a split level and we will need assistance getting him on the first level. Up 5 stairs once on that level I have already gotten a hospital bed and everything he may need. I have even gotten in home care tentatively set up.
How do I get him home? Who do I reach out to? Thanks

If dad is going home on hospice they will provide the transportation.

My question is, jow old is Dad? If you need a transport that means he will be bedbound? Are you really ready to care for someone in this condition? Me personally would not care for a man not my husband if I was going to be changing depends or bathing him. Are you going to have aides come in?

The rehab should be able to help set up transport home with a private medical transport company .
Dad may have to pay out of pocket for it though . Talk to the social worker at rehab .

He is in rehab.
Have they not contacted you regarding "Discharge Planning"?
That is required by LAW in most states.
Call tomorrow.
Ask for whomever is in charge of discharge planning. This is usually a social worker or RN who does only this.
Discuss ALL needs and especially discuss the possibility of coming home on Hospice care. This would allow a hospital bed and equipment free and give you three days a week baths, RN assessment once a week, access to clergy and social worker.

Good luck. Please now discuss ALSO with the doctor the prognosis for your father, and the possibility of Hospice care.

As far as your question regarding transport home, they will assist with this as well. They often have good van transport. They can check your supplemental for other options and also assets in your area that assist with this work.

It’s worth asking if hospice will cover the transport home if he starts hospice while at the rehab.

Most if not all ambulance services offer transport services, so just call the ones in your area.
Hopefully you're not biting off more than you can chew.

Search "Medical transport companies near me"+(zip code of his house). Not sure if this is covered by his Medicare (if he's on it yet) or any supplement he may have. Not sure they will carry him up the stairs so be sure to discuss this when you call. So sorry about your Dad.
