From reading reviews I feel there's a lot risks in selecting a HCP provider where there doesn't seem to be any accountability and is up to a client, the elderly and vulnerable to report abuse in the sector. What's in place as protective measures?

If you mean home care provider such as a CNA... If they work for an agency and they have inappropriate behavior, the agency is responsible for them as they hired and bonded them.

I've been going to doctors and clinics for decades and never once have encountered a HCP who wasn't subject to accountability.

If your HCP is connected to a clinic, you report them to the clinic admins or partners. You can report them to the licensing body who oversees their specialty or the AMA.

If in a facility, you first contact the admins; next the ombudsman; next the licensing board; and then there's always APS.

What types of abuse are you meaning: physical? financial?

In terms of protecting against physical abuse cameras can be installed; see if they have a PoA and if not recommend them to social services for a legal guardian; visit them often. chat with them and look them over for any signs of abuse. Other medical professionals and facility staff are mandated reporters, so having more than one set of eyes on a person is also helpful.
