I wanted to post the letter which contained no confiential information, but i don't see an ability to link. They refer to it as GUIDE and specificially mention Alzheimer support.

I couldn't sign up my mom because she didn't have dementia in her medical records and, when I was looking into GUIDE a few months back, I wasn't ready to move her primary care to a provider within whichever organization we'd sign on with.

But it sounds like a wonderful program; although respite was frequently mentioned by the two organizations I spoke with, training and other support made it sound like a winner. I'd be happy to share my notes by DM.

You can look up participating organizations in your area from the list on Medicare's website at https://www.cms.gov/priorities/innovation/innovation-models/guide. One thing I found was that organizations just starting up the program weren't very helpful; the ones that could competently speak about GUIDE were the ones who'd been at it a few years.

I took from what I read about it after you posted the first time re GUIDE is that there is no financial compensation for the caregiver which is what many come to the forum looking for.
Just wanting to clarify that.

It is a help in finding services that have a monetary value and helpful to the recepient and caregiver but the funds go to the providers to pay for their services. So much per patient they sign up if I remember correctly.

Also a deadline has passed for new providers to join in the program while it is in this current phase, so it’s not like you can ask your current provider to join it. My LO has passed now but I would not have wanted to give up her provider either. Plus she was in a SNF the last three years, so it would have been too late for her.

I do wish someone who is using it or has used it would post as it might be a great help to the caregivers of the newly diagnosed who come here looking for help and aren’t ready to place their loved ones in a SNF.

You can SOMETIMES get help from Medicare, but it isn't much help usually, and it is available only dependent on your not having Advantage type supplemental insurance plans (with those any help available usually comes through your supplemental plan).
MediCAID often has more choice in plans and that is according to the state policies as Medicaid is a combination federal and state program.
