I am currently in a Post-Acute Rehab Facility in San Pedro, CA. I am due to be released any time after June 11th 2026. My Social Worker and I are avidly searching for an Assisted Living facility somewhere near Yucaipa CA that accepts ALW's (I was recently approved!). Can you help me?

This is a Medicaid program. The Social Worker should be able to call the ALs in your area and ask they except Assisted Living Wavers. Does the Waver allow you outbof the county? Then like said, a bed/room has to be open. ALs usually only take a % of Medicaid waver residents.

Have you reached out to your local Agency on Aging? Do you get any benefits?

Way I understand it, the waiver will pay for medical needs but not room and board. So you're looking for a facility that not only will accept you waiver, but also accept and have "rent" close to what you're bringing in per month to cover room and board. ( ie SSI, SSDi, etc)
