"Because North Carolina is not an "income cap" state, Medicaid applicants are not automatically disqualified if their income exceeds a certain limit. Instead, NC utilizes a "spend-down" pathway. From what I understand not being a "income cap" state, if all they have is Social Security and maybe a pension Medicaid will except that. All other assets will have to be used towards their care. Yes, an Elder Lawyer can help.

Hire an eldercare attorney. The cost will be paid by her and is an allowed spend down expense.
